The Interrogators - As Casper examines Izzy's dead body in front of Steed & Mother the boom mike appears several times at the top of the screen.
Legacy of Death - when Tara makes her first appearance in the episode at Steeds door you can clearly see that Linda Thorson's right hand has been injured and is heavily dressed in a flesh coloured bandage. However a few seconds later after pouring a glass of champagne and handing it to Steed the bandage is missing.

Oops, I found an interesting mistake in the episode The Living Dead just recently.
After fighting in the prison cell, Emma straightens up and looks at the bodies of Masgard and Tom (off-screen) lying on the bed.
...
...
In the next footage (pic below), when the film camera shows these bodies of Masgard and Tom on the bed in the average plan, Emma mysteriously disappears from her place (location).
In subsequent footages, Emma again in its place on TV screen.
Maybe she used the "invisibility" formula.
After fighting in the prison cell, Emma straightens up and looks at the bodies of Masgard and Tom (off-screen) lying on the bed.

In the next footage (pic below), when the film camera shows these bodies of Masgard and Tom on the bed in the average plan, Emma mysteriously disappears from her place (location).

That is, it was not necessary to show the viewer a bed and two bodies in average plan in a separate footage between two footages with large plans of Emma. There should was to be another insert (maybe close-up) or no insert at all. So there it would be possible not to cut in half the footage:
Looks like wrong editing to me, as is often the case in the TV series, which catches up with the production schedule.

Right now I watch the episode The Hidden Tiger, found a new mistake.
When the camera shoots Bellamy's body in the average plan, he lies in torn clothes with a bandage around his neck next to the brick support of arch painted black and white...
...but when the camera takes his body close-up, the Bellamy's bandage is a tie shape on his chest, and the brick support of arch is painted only in white.

If you watch the whole sequence it's obvious that Bellamy is wearing a blue and purple silk cravat (and it's just as obvious when the shots are of the studio set of the porch rather than location footage).
